Changing Your Name on Your Social Security Record
The SS-5 Application for a Social Security Card form is used to change your name on your social security record. Note that your card only lists your name; gender markers are kept in a computer file.
To change your name on your social security record, you need:
- Name Change Order
Submitting this form:
Fill out the SS-5 Application for a Social Security Card
- We recommend submitting this form and the required documents in person at your local Social Security office
- If you choose to mail the documents, you can find your local office on the Social Security website.
